The Best
Benefits Discount Medical Plan Card provides valuable discounts and can
complement other Health Care Plans. Best Benefits plan is not insurance.
However, using Best Benefits plan providers can substantially reduce
the direct cost of doctor visits, hospitalization, prescription drugs, lab
work, dental treatment and vision care and a wide variety of other medical
expenses.
Best Benefits plan members can receive care
at preferred rates at over 300,000 participating providers nationwide.
Cardholders receive discounts without the necessity of special forms.
The Best Benefits plan portfolio
of providers and services includes:
- Physician services - 5 to
40% savings
- Lab
work services - average savings of 70% for most common tests
- Hospital - 5 to 40% savings
- Prescription drugs - up to
30% savings on generic and brand name drugs
- Dental services - 10 to 50%
savings
- Vision services - 10 to 60%
savings on designer frames, bifocals and contact lenses
- Chiropractic care - 20 to
40% savings
- Optional tiered Prescription Savings Plan

Fees
This Best Benefits discount medical plan card saves you money when you use it with
in-network medical providers, including physicians, hospitals, and lab work
providers. Additional networks for pharmacy discounts, dental care discounts,
vision care discounts and hearing care discounts are included with the
basic discount medical plan card. An optional Prescription Savings Plan is available to
benefit individuals who are on continuous medication.
Member fees
for the basic discount medical plan card are:
$16.95 per month for an individual
$19.95 per month for your immediate family
The optional
Prescription Savings Plan fees are an additional:
$14.95 per month for an individual
$19.95 per month for your immediate family
Plan
participants will receive a Best Benefits Plan discount medical plan ID
card. With the ID card, a brochure will be provided which explains the
discounts available, how to access the discounts available and a
personalized listing of local participating providers. Participants may
also search for participating
providers for the discount services available by visiting the Best
Benefits plan website (www.findbestbenefits.com); enter the member code and
follow the onscreen instructions.

insurance. The plan provides discounts at certain health care providers
for medical services. The plan does not make payments directly to the
providers of medical services. The plan member is obligated to pay for all
health care services but will receive a discount from those health care
providers who have contracted with the discount medical plan organization.
The range of discounts for services offered will vary depending on the type
of provider and service. Discounts on hospital services are not available in
Maryland. This is not a Medicare prescription drug plan. This plan is

effective date for a full refund of fees paid. This Plan is not available in
