Patient Directed Care (PDC) is presently building
a Provider Service Network for 14 counties in the State of Florida
for the purpose of serving Medicaid TANF and SSI eligible clients. The PDC
Provider Service Network (PSN) will be based on a Medical Home Model of
primary care physicians and specialists as well as hospitals and other
medical providers.
PDC is dedicated to creating a patient
friendly health care system that delivers high quality cost-effective
health care within the State of Florida
and communities it serves. In bringing together health professionals
in the local communities, we will jointly create an environment in which
information is transmitted quickly and efficiently in order that optimum
patient care is achieved in a cost effective mode.
The value proposition of the PDC PSN medical
home model is: Investing substantially more in
primary care, improving the infrastructure of medical practices, and
increasing compensation for primary care and specialist providers will
yield better clinical outcomes and constrain the rate of rise in healthcare
spending in the long term.
